1. The Client has consulted and been advised on the General Terms and Conditions for Services provided by Citispa, Separate Terms and Conditions for each special Service Package/Treatment attached to this confirmation (if any), and Citispa’s internal regulations on operation and management issued from time to time.
2. Client’s information and images will be saved in Citispa’s system for management purposes.
3. If the Client/Payer requests the Company to issue VAT invoices to another unit, the Client/Payer understands and agrees that any issue arises during the performance of this Contract which leads the Company to refund part or all of the service fees of this Contract.
The Company will work directly with the unit whose information for issuing VAT invoice was provided by the Client/Payer to adjust the corresponding VAT invoice. The Company will refund the relevant service fee to this unit in accordance with the tax laws of Vietnam.
The Client/Payer is responsible for working with the above unit to receive the service fee refunded by the Company. The Company will not be liable if the Client/Payer does not accept or receive an inadequate refund of the service fee from the above unit.
4. This Confirmation shall take effect from the date of signing until the end of December 2026 and shall be binding the Client upon each and all Service Package/Treatment used during the above period of effectiveness.
5. This Confirmation is governed by and construed by the laws of Vietnam.
6. In the event of any dispute arising out of or in connection with this Confirmation or the Service Package/Treatment purchased by the Client, such dispute shall be resolved by the Client and the Company through negotiation and conciliation on the spirit of cooperation. If the dispute cannot be resolved through negotiation or conciliation, within thirty (30) days from the date the dispute arises, the Client or Citispa may request a competent Court to resolve the dispute.
7. This Confirmation, internal regulations and other provisions issued by Citispa from time to time contain the entire agreement between the Client and Citispa about the Services and replace any agreement, negotiation or previous oral communication with the Services.

9. This Confirmation is made into two (02) originals in English, having the same legal effect. The Client and the Company shall keep one copy each for performance.
